Ozactin Liability Waiver

I acknowledge, understand, and agree to the following:

1. Informed Consent - I am voluntarily purchasing a dietary supplement that is intended for administration via Nebulization. I understand that this method of use may carry health risks and have not been evaluated by the U.S. Food and Drug Administration (FDA) for safety or effectiveness in treating, curing, or preventing any disease.
2. Assumption of Risk - I understand that:
1. The use of supplements via IV, topical, or nebulized delivery can involve risks such as allergic reactions, infection, irritation, adverse effects, or unknown complications.
2. I assume full responsibility for my choice to purchase and use the product in any of the intended methods, and I understand that the outcome is not guaranteed.
3. Medical Disclaimer - 
   I acknowledge that this product is not a substitute for medical care or advice from a licensed healthcare provider. I have consulted, or will consult, a qualified practitioner before using this product, especially through non-oral routes such as IV or nebulization.
4. Release of Liability
   - By completing this purchase, I agree to release, indemnify, and hold harmless the seller, its owners, affiliates, employees, agents, and manufacturers from any and all liability, claims, damages, or legal action resulting from my use, misuse, or handling of the product, whether foreseeable or not.
5. Legal Capacity
   - I affirm that I am 18 years of age or older, purchasing this product of my own free will, and legally authorized to provide this consent and agreement.
6. Digital Agreement - 
   I agree that by checking the box, this constitutes a legally binding electronic signature and acknowledgment of the terms of this waiver, in accordance with applicable electronic signature laws (such as the U.S. E-SIGN Act and UETA).
